Living Well with Dementia in the community.
The Dementia Support group has been developed to bring Dementia specific support to the community.
Group sessions will include:
- Namaste – a person centered therapeutic approach which aims to promote improved quality of life for people in the later stages of dementia.
- Memory Café – a relaxed environment to socialise and discuss times gone by.
Carers are welcome too!
If our community based support isn’t suitable we currently provide Dementia Support at the Hospice as well. We hold dementia friendly therapeutic groups within The Living Well Centre including Cognitive Stimulation Therapy groups and a Sporting Memories Reminiscence group. Whilst our Namaste Care Service can provide home based sessions for people with advanced dementia who may benefit from multi-sensory stimulation.
If you would like to join our Namaste please review the relevant referral criteria and referral form. Or if you would like to have an informal conversation about the groups please contact us or even drop in to our Hospice Hub Coffee Morning for a cuppa and chat. You do not need to fit a specific criteria or make a referral for the drop in Memory Café sessions.
How to refer into the Community Outreach Services
Application to attend the support groups must be made by referral from professionals based in the community or via self-referral. Our Community Outreach Services will follow up with a short telephone assessment within 15 days of the referral.
