Living Well with Illness programme.
The Living Well with Illness programme will initially run for 6 weeks as a part of our 6 week Community Outreach Project pilot.
Every Wednesday – 1pm-2:30pm – Auckland Communal Room, Auckland, Waldridge, DH2 2UA.
Following the pilot it will run for 10 weeks.
We have developed this programme to support those living with a life limiting illness in the community. The course will be delivered as a social and educational programme addressing themes such as:
- Fatigue and sleep management
- Nutrition and hydration
- Strength and balance
- Relaxation
As well as the above themes, the programme will include activities, guest speakers and plenty cups of tea.
We also offer a Living Well with Illness programme within our Hospice Living Well Centre for those who may require more clinical support based on their individual needs.
How to refer into the Community Outreach Services
Application to attend the support groups must be made by referral from professionals based in the community or via self-referral. Our Community Outreach Services will follow up with a short telephone assessment within 15 days of the referral.
